Abstract
Treatment of diverticulitis: evidence for new guidelines Diverticulitis is a common complication of diverticu- losis, usally occurring in the sigmoid and the left colon. Recently, evidence has arisen for a less aggressive approach in the treatment of both com- plicated and uncomplicated diverticulitis. Acute uncomplicated diverticulitis in patients with- out severe comorbidity responds equally to ambulatory treatment (oral antibiotics and easily digestible foods) as to the classic scheme of intravenous therapy and bowel rest. Moreover, recent evidence questions the use of antibiotics in acute uncomplicated diverticulitis altogether, though this remains a controversial issue. In recurring uncomplicated diverticulitis, a resection following two episodes is no longer the norm. A more individualized approach based on comorbidity and fre- quency/severity of the episodes is preferred. Even in complicated diverticulitis, this less aggressive trend is visible: abscesses up to four centimeters may be treated conservatively, while percutaneous drainage is an option for the larger ones. Only when these approaches fail, surgery has to be considered; primary anastomosis is preferred over a Hartmann’s operation.
